For Patients:
1.
A general brochure about Part D is available from Medicare called
Medicare Covers America  upon which you or a staff member can write
the phone number for
NC SHIIP (1-800-443-9354) or the patient's specific
Local SHIIP coordinator and their Local Outreach Coordinator.

2.  The excellent website My Medicare Matters from the National Council
on the Aging really helps patients understand Part D, answers frequently
asked questions and guides them through the enrollment process.  
(Click "Increase Text Size" for seniors who may need this feature.)

3. NC SHIIP's "Just the Facts"brochure for NC Medicare beneficiaries, it
is just one page front and back, but is in very small print.

4. One particularly informative handout to print out for patients called
Five Main Audiences,  (from CMS) explains how decisions should be
made based on one's current prescription drug coverage situation.

5. Tip Sheet from NC SHIIP helps NC seniors shop for a Part D plan
(1pg).
For Doctors:
1. ePocrates has made all of the Part D formularies (PDPs and MA
PDPs) available for
free download to your desktop or Palm Pilot or
Pocket PC.  This is by far the easiest way to look up whether a drug is
covered in a patient's new formulary, at what level/tier it is covered, and
what similar alternatives exist if it is NOT covered.

2.  CMS Provider Resource Center has several resources, including a fax
form for communication w/Pharmacists about changing drugs within a
class to fit a patient's new formulary.  Note that if prior authorization is
needed or if you need to request a medically necessary exception to the
patient's new formulary, the patient's plan must be contacted directly.

3. The Medicare Rights Center's Prescription Drug Helpline  for medical
professionals helps answer providers' Medicare Part D questions.

MedicareRxConnect is a web resource designed by patient advocate
organizations to help Medicare beneficiaries and their caregivers to
quickly find the information they need (including state resources available
to them) about the new Medicare Prescription Drug Program.

Center for Medicare Advocacy is an experienced Medicare Advocacy
group that has resources at this link to help folks understand Part D.

CMS MedLearn is a website that includes posters, audiovisual
presentations, links, and tools to help physicians and their patients
understand Part D.  CMS has brochures about every special group one
could name at:
